Externship Forms


Placement Preference Form

Externship Placement Preference Form - This form must be completed and submitted with all externship application(s). Students may apply for up to 5 externships per semester.

Application Forms

Domestic Violence, Family Law and the Florida Supreme Court externships require additional application forms. The following forms must be submitted with applications in addition to all other required documents.

Other Required Forms

Externship Program Acknowledgement & Registration Form - Students must complete this form when an externship is accepted. The form must be picked up in the Dean's office prior to pre-registration. Externship registration will not be processed until this form has been completed.

Externship Time Log - After an extern begins an appointment, time logs must be submitted to the Dean's Office on a bi-weekly basis.

Evaluation Forms

Student's Evaluation of Field Placement - To be completed by the student at or near the end their externship.

Field Instructor's Extern Evaluation - To be completed by the Field Supervisor at or near the end of the student's externship.
